Limited Authorization to Prepare and File Settlement Claims
1. What you authorize ClaimWisely to do
By accepting this authorization, you appoint ClaimWisely as your limited, non-exclusive administrative representative to:
- Review publicly available class-action settlement programs and assess your potential eligibility using the information you provide;
- Prepare, complete and electronically submit claim forms, certifications and supporting documentation to the relevant settlement administrators;
- Respond to administrator requests for corrections or additional information; and
- Track filing deadlines, claim status and distribution schedules for submitted claims.
2. What this authorization does NOT include
- Legal advice, legal representation or an attorney-client relationship — ClaimWisely is not a law firm;
- Filing, prosecuting or settling lawsuits, or appearing in court on your behalf;
- Negotiating with defendants or their counsel; or
- Receiving, endorsing, cashing or taking possession of your settlement payments. Awards are paid by the administrator directly to you.
3. Your responsibilities
You confirm that the information you provide is true and correct to the best of your knowledge, that you will promptly update it, and that you understand claim forms and certifications may be made under penalty of perjury. You are responsible for the factual accuracy of every claim submitted on your behalf. You should review any submission you are asked to confirm.
4. Term and revocation
This authorization remains in effect while your membership is active. You may revoke it at any time by emailing support@claimwisely.org. Revocation stops future filings; claims already submitted to administrators remain with those administrators under their own procedures.
Acknowledgment: ClaimWisely provides administrative assistance only. Decisions about eligibility and awards rest exclusively with settlement administrators and courts, and no specific outcome or payment is guaranteed.
